Rheumatoid Arthritis is a chronic illness that affects the joints, mostly small joints of the hands and feet, but can also affect the big joints. It may cause swelling of the joints, a lot of pain, and morning stiffness, which, if not treated, may lead to joint deformities over a period of time. This disease can also affect the eyes, lungs, skin nerves, and other parts of the body.

In earlier days, there were limited options to treat this condition. However, with the advances in medicine, we now have many more options to treat this condition and prevent deformities from happening.

These medications include conventional disease-modifying anti-rheumatic drugs such as Methotrexate and folic acid, sulfasalazine, hydroxychloroquine, azathioprine, etc.; Biologic medications such as Humira, Enbrel, Simponi, Cimzia, Remicade, Orencia, Actemra, Xeljanz, Rinvoq, etc.
The acute flares can still be managed by prednisone (oral or injections).

Lupus is a systemic disease that can affect many parts of your body. It commonly affects skin, kidneys, and joints and can give rise to symptoms such as butterfly rash on the cheeks, oral ulcer, hair loss in patches, fatigue, fever, pleuritis, pericarditis, seizure, clots, multiple miscarriages, and many more. It is diagnosed by blood tests and may need a biopsy of the involved organs.

There are good treatments available nowadays, including hydroxychloroquine, Azathioprine, Methotrexate+Folic acid, Belimumab, Mycophenolate, Voclosporin, and, for severe cases, Rituximab and Cyclophosphamide. These medications are high-risk medications and have side effects that your doctor will discuss with you before starting you on any of these.

Sjogren's syndrome is a connective tissue disease that can cause a lot of dryness in the eyes and mouth and can also cause multiple joint pains. It can also affect the lungs and kidneys, and sometimes, it can also affect blood vessels.

It can be diagnosed with clinical exams, blood tests, and sometimes biopsy. There are several treatments available for it.

It is recommended that you get screened for lymphoma if you have Sjogren's disease. It can be done by a simple clinical exam and a blood test.

Osteoarthritis is a degenerative type of arthritis that results from the wear and tear of joints. If we are fortunate enough to live a long life, most of us will experience this disease as part of aging. Psoriatic arthritis is a type of inflammatory arthritis that is often seen in patients who have the skin disease Psoriasis. Psoriasis can present as silvery scaly rashes. It can affect your skin and nails. Oftentimes, when there is nail involvement, there may be bone and joint involvement as well. It is diagnosed with the help of clinical exams, blood tests, and imaging.

Fibromyalgia is a disease that can cause pain in multiple parts of the body and is identified by the presence of tender points. It can be present in the presence of other autoimmune diseases. Oftentimes, another diagnosis, such as persistent insomnia, depression, anxiety, overactive bladder, and irritable bowel symptoms, can accompany the diagnosis of Fibromyalgia and may contribute to the presenting symptoms.

The diagnosis of Fibromyalgia brings good news and bad news. The good news is that in the absence of connective tissue diseases, it does not affect longevity and doesn't damage any vital organs of the body by itself. The bad news is that it can significantly affect the quality of life, and the treatment options are limited.

Medicines such as duloxetine, gabapentin, pregabalin, SSRI, SNRI, and Amitriptyline are some of the options that may help. However, behavioral therapy, including mild aerobic exercises, aquatic exercises, and meditation, plays an important role in the outcomes. Gout can present as a sudden onset of severe pain in one or more joints. It most commonly affects the first toe and can also affect other joints such as ankles, knees, hands, etc.

Gout can be prevented to a certain extent by avoiding seafood, red meat, alcohol, soda, and high fructose corn syrup.

Tea, coffee, orange juice, and water are considered safe drinks for Gout.
Gout can be treated with medicines like Allopurinol, Febuxostat, Pegloticase, and others and usually needs monitoring of Uric acid along with routine labs.

Compliance with medicine is very important. We advise you not to make any changes to your Gout medications without a discussion with your doctor because any change in the dose without appropriate treatment may precipitate Gout.
